Hello, you have to connect the mouse and keyboard to the titan two, without any hub. And use an OTG cable to plug the third controller in the PROG port for authentication.
Console tuner since my 1st controller.

What is your keyboard? When you plug in the Titan Two does the led lights green? Did you tried an other usb port?

I can't help for that, i don't use that feature because my headset have a usb receiver plugged into the console.
But did you do the first config?

First-time Configuration

On PlayStation 4, enable wired communication:
Settings → Devices → Controllers → Communication Method: Use USB Cable.
Configure the Output Protocol of the Titan Two to USB Automatic or USB PlayStation 4.
Console tuner since my 1st controller.
Scripting, a game in the game.

For the headset...
headsets that plug in into the controller are supported only in non crossover mode..
and for the PS4 it requires a "DualShock 4 V2" controller as stated on this page, specific features/Speaker+Headset:
https://www.consoletuner.com/wiki/index ... c_features

I think the v2 (2018?) are the one with the lightbar visible through the touchpad, the ones shipped with the ps4 pro and ps4 slim if I remember correctly.

Yes i forgot to say that a DualShock 4 V2 is required for the headset support as well as the controller's speaker.

Giuseppe90 - have you installed the input translator in the slot?

Not sure if it will change anything, but try connecting like this:

Input A: Mouse
Input B: Controller
Prog: Keyboard with OTG adapter
Output: USB cable to PS4

If you have a computer nearby, and the keyboard you intend to use is plugged to the PC you can use Gtuner's KMG Capture feature for Windows OS.

Then in this case you could do:

Input A: Mouse to connected to Input A or connected to PC [I believe]. Do not forget to configure the DPI values in Gtuner.
Input B: Controller
Prog: Connected to PC
Output: USB cable to PS4

Perhaps this thread may help you, too:
