Progress, I have finally got it to work after changing cables and controllers multiple times. The only issue now is the controller seems to disconnect frequently. I have added powered USB hub between console and T2 but still same issue, every few minutes the controller disconnects and I lose control.

The issue definitively seems to be power related (lack of power). With XIM/Apex there will be at least 5 devices connected at the same time (Titan Two, XIM/Apex, controller, keyboard and mouse), this configuration can potentially require more power than what the USB port from the console can supply.
The powered USB Hub can help if connected on XIM/Apex to provide connection and power for the keyboard, mouse and controller. In this configuration the console needs to power only the Titan Two and XIM.
However, the best powering solution is using the external power supply on the Titan Two.

I have folloeing setup:
M&K + PS4 Controller -> Xim apex -> T2 -> AC powered USB hub -> PS4 console.
I thought it was risky to connect T2 directly to AC and it was better to connect to USB powered hub, based on psts I have read? Should I add a second powered usb hub to set up?

Try use the USB hub you already have in this configuration:
K+M+Controller -> USB Hub -> Apex -> TitanTwo -> PS4
