After turning on PS4 and booting the BO4 game, I got an E 4 error. I was able to get it working again by disconnecting and reconnecting.

Thank you for making the video explaining the connections.

E4 is out-of-memory error. Happens when the Titan Two tries allocate memory to store some data and there is no enough memory available.

99% of the time this is caused when the plugged controller is not capable to receive the led/rumble packets (or audio packets) as fast as the packets are generated. The exceeding packets are then cached by the Titan Two to be send when possible. If this situation persists for certain amount of time the memory will be totally filled with cached packets, ending with E4 error.

Now, the most common cause for this are scripts that make wrong use of set_led() and/or set_ffb().

My question is: were you running a script when this error happened? if yes, please post here (or PM me) the script code, so I can check.

Thank you.

No, I was not running a script. I had just booted up the PS4 and the system was turning on. I have powered off the rebooted the setup multiple times since the video without any issues. I have also been playing for a few hours without any issues. It may have just been a weird glitch on after updating. I will keep testing.

I played for a few more hours without any issues. Fingers crossed.

It has now been several days of playing and I have not experienced any issues. It seems to be fixed.

I stand corrected. It has all the sudden started acting up again. When it happens now the lights on the DS4 are off and it won't sync back up. I often have to unplug everything and reconnect in order to get it to sync. Sometimes, I have to do it a few times. I played this entire weekend with the DS4 on Input A without any issues; therefore, the problem does NOT seem to be my cable or DS4. I am going to get a new one just in case.
