A Bluetooth connection with the Titan Two to fetch relevante info to display on the OLED display seems the best option. To power the eletronics of your adapter, not sure what would be the best option. But it would be nice avoid the needed of two usb cables, maybe an usb adapter just to get power from the cable connected on the controller.
I already ordered a bluetooth module for the teensy board, can't wait to test it as soon it arrives.
Great idea to power both with one cable... going to try that one first ;-)

Re: More buttons ? -> Custom DS4 snap-on device
I have improved the design a bit
And I have uploaded all the files for 3d printing and a description how to build this thing yourself to thingiverse :
https://www.thingiverse.com/thing:2722940
For printing you need the file "_all_parts.stl" and get the additional parts listed in the things detail page plus following the instructions in file _assembly_instructions.pdf if you feel adventurious enough to build your own ;-)
the vertical stand is also published:
https://www.thingiverse.com/thing:2722858
You can use the https://www.3dhubs.com/ service to find a store near your location to get the files printed if you don't own a 3d printer yourself

looking forward to updates on this project Scachi, especially regarding what bluetooth module you used for the teensy and how you ended up powering it
